Cancer Caregivers terms and conditions.
- Cancer Caregivers is available to inforce Fidelity Life trauma, life or income protection cover customers who are taking care of a family member or close friend with cancer.
- The service is also available to immediate family members when the customer has either a life, trauma or income protection cover in place and has a cancer diagnosis. Cancer Caregivers is not available for lapsed policies, or policies subject to cancellation requests.
- Immediate family members are defined as the customer’s spouse or partner (including civil union and de facto relationships), living in New Zealand, and the following:
• biological or adoptive parents, children or siblings
• step-parents and step-siblings, provided they have lived as part of the family unit for a predominant period of the person's life
but expressly excludes spouses, civil union partners, and de facto partners of any of the above listed family members. You must be at least 18-years old to use the Cancer Caregiver services. - Your enrolment in Cancer Caregivers is separate from any claim and you do not need to have a valid claim under your policy in order to be able to access these services.
- Cancer Caregivers is not available on Group covers, Westpac life insurance policies or any other cover types, e.g. Permanent Disability cover, or Key person covers.
- Although Osara Health will be your relationship providers for these support services once you have enrolled in Cancer Caregivers, the relationship for your policy and your claim remains with us.
- Cancer Caregivers is not a policy benefit and Fidelity Life reserves the right to discontinue it or amend the eligibility criteria or these terms and conditions at any time. If you have already commenced participation in Cancer Caregivers and we elect to suspend or discontinue it, we will work together in good faith with Osara Health to ensure that you will continue to be supported until the conclusion of the services.
- Fidelity Life collects, holds and uses your personal information in accordance with the Privacy Act 2020 and our Privacy Statement. You have the right to request access to, and correction of, any personal information held by Fidelity Life.
- For customers enrolled in Cancer Caregivers, your first name, last name, date of birth, email address, phone number and policy number will be disclosed to Osara Health as our third-party service providers in accordance with the terms of our Privacy Statement, and you consent to the use of your personal information for this purpose.
- Except for liability that may not be excluded by law, Fidelity Life will not be liable for any loss, claim, cost, expense, liability or injury suffered by you in relation to the provision of Osara Health’s services. You should always consult your treating doctor on any assessment.
